ABSTRACT

Objective:To explore the difference in antibacterial mechanism between <italic>Coptis chinensis</italic> and<italic> </italic>its<italic> </italic>flower stalk based on secondary metabolites and network pharmacology. Method:Based on the ultraperformance liquid chromatography mass spectrometry(UPLC-MS/MS) detection platform,the secondary metabolites database of <italic>C. chinensis</italic> and its flower stalk(MWDB) was built. The common database of metabolites information and the multivariate statistical analysis were used to study the differences of secondary metabolites between <italic>C. chinensis</italic> and its flower stalk and screen out 18 metabolites of<italic> </italic>the<italic> </italic>flower stalk and 11 metabolites of <italic>C. chinensis</italic> with a high content. BATMAN-TCM database was used to obtain the targets of component action,and their corresponding genes were inquired in the UniProt database. GeneCards was retrieved for antimicrobial genes,and the intersection genes of components and antimicrobials were obtained on Venny platform. Through DAVID gene ontology(GO) enrichment analysis and Kyoto encyclopedia of genes and genomes(KEGG) pathway enrichment analysis,the mechanism of its action was predicted,and the results were visualized through histogram and advanced bubble diagram drawn by GraphPad Prism software and OmicShare database. The protein-protein interaction(PPI) network was constructed by STRING, database and the component-target-pathway network was constructed by Cytoscape 3.7.2 software. The antibacterial differences were compared based on the results of network pharmacology analysis. Result:Through network pharmacology,the antibacterial active components of <italic>C. chinensis</italic> were 5 fewer than that of the flower stalk,55 more antibacterial targets than that of the flower stalk; quercetin and berberine were predicted to be the common components of the antagonistic action of <italic>C. chinensis </italic>and the flower stalk. Key genes involved in antimicrobial action were p38 Mitogen-activated protein kinase 14(MAPK14),catalase(CAT); malaria and Toll-like receptor signaling pathway were different key pathways involved in antimicrobial activity. Conclusion:<italic>C. chinensis </italic>and the flower stalk mainly exert the antibacterial effect in a multi-target and multi-pathway manner,which can offer new ideas and clues for the study of antibacterial mechanism of<italic> C. chinensis</italic> and the flower stalk,and provide a new development direction for the comprehensive development and rational application of the flower stalk resources.

ABSTRACT

Objective:To study the protective effect of the Wenyang Huoxue Huatan prescription (WYHXHT) on cardiotoxicity induced by adriamycin. Method:SD rats were randomly divided into the following six groups: a normal control group, an adriamycin model group, a low-dose (4.86 g·kg<sup>-1</sup>) WYHXHT group, a middle-dose (9.72 g·kg<sup>-1</sup>) WYHXHT group, a high-dose (19.44 g·kg<sup>-1</sup>) WYHXHT group, and a dexrazoxane group. Except for the normal control group, the rats in other groups received intraperitoneal injection of 2.5 mg·kg<sup>-1</sup> adriamycin, once a week for six weeks, with a cumulative dose of 15 mg·kg<sup>-1</sup>. The normal control group, the adriamycin model group, and the dexrazoxane group received 10 mL·kg<sup>-1</sup> normal saline daily by gavage. In the dexrazoxane group, the rats were subjected to intraperitoneal injection of 25 mg·kg<sup>-1</sup> dexrazoxane 30 min before doxorubicin administration, once a week for six weeks. The general condition of rats was observed and their body weight was monitored. A high-resolution micro-ultrasound imaging system was used to detect rat cardiac function. Hematoxylin-eosin (HE) staining was performed to observe the pathological changes of myocardial tissues of rats. Western blot was used to detect the protein expression of microtubule-associated protein 1 light chain 3 (LC3) Ⅱ, the mammalian homolog of yeast Atg6 (Beclin-1), and p62 protein in rat myocardial tissues. Result:Compared with the normal control group, rats in the adriamycin model group showed dull fur, reduced food intake and activity, loose stool, low energy, and slow response. Besides, it also displayed reduced body weight (<italic>P</italic><0.01), decreased left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) (<italic>P</italic><0.01), myocardial cell degeneration, edema, rupture, and dissolution, expansion of myocardial interstitium, uneven staining of myocardial fiber, visible inflammatory cell infiltration, up-regulated expression of Beclin-1 and LC3Ⅱ in rat myocardial tissues (<italic>P</italic><0.01), and down-regulated p62 expression (<italic>P</italic><0.01). Compared with the adriamycin model group, the medium- and high-dose WYHXHT groups exhibited increased body weight, LVEF, and LVFS (<italic>P</italic><0.01), relieved pathological injury of myocardial tissues, down-regulated expression of LC3Ⅱ and Beclin-1 (<italic>P</italic><0.05,<italic>P</italic><0.01), and up-regulated expression of p62 (<italic>P</italic><0.05,<italic>P</italic><0.01). Conclusion:WYHXHT can effectively prevent and treat adriamycin-induced cardiotoxicity, and its effect may be related to the inhibition of myocardial cell autophagy. The effect is dominant in the high-dose group.

@#Objective    To evaluate the mid-term results of surgical treatment for functional single ventricle associated with total anomalous pulmonary venous connection. Methods    We reviewed the clinical data and follow-up results of 12 patients in our hospital who underwent both single ventricle series palliation and total anomalous pulmonary venous connection correction from 2008 to 2018. There were 6 males and 6 females at age of 2.3 (1-21) years. Univariable and multivariable Cox proportional hazard regression methods were performed. The Kaplan-Meier method was used to estimate the survival rate. Results    All patients were successfully removed from extracorporeal circulation. The cardiopulmonary bypass time was 113.8±42.5 min. The myocardial block time was 57.7±31.7 min. There were 3 in-hospital deaths and 4 late deaths. The causes of death in hospital were pulmonary hypertension, pulmonary hemorrhage, and hypoxemia. During follow-up, the cause of death was heart failure. The survival rate at 1 year and 3 years was 58.3%(95%CI 27% to 80%) and 40% (95%CI 13% to 65%), respectively. Median follow-up was 48.3 months (range: 1 to 118 months). Conclusion    Functional single ventricle combined with complete pulmonary venous drainage is a serious clinical disease, with poor therapeutic effect and high overall mortality.

@#Objective    To analyze the clinical characteristics and surgical treatment results of cardiac tumors. Methods    Clinical data of 28 patients with cardiac tumors, who underwent cardiac surgery in our hospital from 2013 to 2019 were retrospectively reviewed. There were 12 males and 16 females. The median age was 11.5 years, ranging 1 month to 69 years; and the median weight was 39.0 (4.8-100.0) kg. All patients underwent tumor resection under cardio-pulmonary bypass with general anesthesia, and postoperative pathological analysis was performed. Results    There were 27 patients with primary cardiac tumors and 1 patient with secondary cardiac tumor. Among the primary cardiac tumors, there were 16 myxomas, 5 rhabdomyomas, 3 inflammatory myofibroblastomas, 2 fibroids and 2 hemangiomas. The secondary cardiac tumor was chondrosarcoma. A total of 28 cardiac tumors occurred in 29 locations, including 14 in the left atrium, 3 in the right atrium, 1 in the left ventricle, 7 in the right ventricle, 2 in the mitral and 2 in the tricuspid. Cerebral embolism occurred in 6 patients, peripheral vascular embolism in 3 patients, and syncope in 3 patients. There was no death during the perioperative period and in the long term follow-up. Complete resection was performed in 25 patients and partial resection was performed in 3 patients. During the follow-up, 2 patients with myxoma received reoperation for recurrence. Among the 3 patients with partial resection, residual tumor tissue disappeared in 2 patients, and in the other patient, there was no change in the size of the residual tumor tissue during the follow-up compared to that at discharge. Conclusion    Cardiac surgery is an effective method for patients with primary benign cardiac tumors. However, whether the cardiac tumor tissue can be completely removed depends on the anatomical location of the tumor during the operation. If there are systemic embolism events and syncope, the possibility of cardiac tumors should be considered.

ABSTRACT

Objective:To evaluate the accuracy of measuring the siae of atrial septal defect(ASD) by transthoracic echocardiography(TTE) on " compression of right superior pulmonary vein sheath during operation" (compression-size) using multi-layer perceptron.Methods:From January 2016 to January 2019, 460 cases of ASD with soft margin, thin growth or central type with atrial septal tumor in the First Hospital Affiliated of Air Force Medical University were analyzed retrospectively. The age was (34.32±18.84)years, 129 males and 331 females. Preoperative TTE and intraoperative TTE were performed to measure the size of ASD and compression-size. With the final occluder size selected as the dependent variable, the preoperative and intraoperative ultrasound measurements were used as covariates, and all patients were randomly divided into training set, test set and verification set according to the proportion of 5∶2∶3, which were imported into multi-layer perceptron. Gradient descent method was used to optimize and adjust the weight, and 10 operations were carried out respectively, and the average value was calculated to evaluate the two ultrasound methods for blocking.Results:The mean values of ASD were (15.26±5.33)mm for preoperative, (23.83±6.39)mm for compression-size intraoperative TTE, and (25.14±6.55)mm for the occluder, respectively. The difference was statistically significant (χ 2=850.450, P<0.001). There were significant differences between the pairwise ( P<0.001). Ten times of multi-layer perceptron were used to obtain a training model for predicting the size of the occluder based on preoperative transthoracic ultrasound and TTE measurement after right upper pulmonary vein sheath compression. The average relative error of the model obtained by preoperative ultrasound measurement in the validation set was (16.55±0.02)%, and that of the model obtained by intraoperative ultrasound measurement in the validation set was (4.81±0.01)%. The difference between the two methods was statistically significant ( t=16.185, P<0.001). Conclusions:It is more accurate to measure the ASD size for selection of occluder by the use of the right superior pulmonary vein sheath compression and TTE, especially for ASD patients with soft margin, thin growth or atrial septal tumor.

ABSTRACT

Objective: To establish the quality standards of Reduning Injection (RI) based on UV fingerprint, and improve the quality standards of RI. Methods: Full wavelength UV-vis spectrophotometer was used to scan the 15 batches of RI, a reference fingerprint was established, correlation coefficient method was used to calculate RI sample fingerprint and control fingerprint similarity, and the 14 batches of samples were verified. Results: The five batches of RI were within the validity period, their UV fingerprints were in line with the requirements, similarity was all above 0.902, while the absorbance values were in the ranges of 0.693-0.781 (323 nm), 0.323-0.370 (267 nm), 0.957-1.067 (236 nm), 0.902-1.013 (226 nm), 0.953-1.075 (218 nm), and 0.926-1.052 (211 nm); In addition the expired nine batches of RI, similarity is less than 0.902 or the absorbance value does not meet the standard. Conclusion: UV fingerprint method can be used as a simple and accurate method of quality evaluation for the quality control of RI.

ABSTRACT

The method of physical fingerprint spectrum for Reduning injection (RI) was proposed in this paper to improve its quality standards based on the strong correlation between physicochemical properties of drugs, their safety, effectiveness and stability. The quality of RI was studied by the thought and method of physical chemistry. The physical fingerprint spectrum was visually showed by the radar map, and consisted of eight indexes (pH, conductivity, turbidity, refractive index, osmolarity, surface tension, relative density, and kinematic viscosity). Then 12 batch of samples were verified. It was found that the physical fingerprint spectra of 3 batches of RI were in line with the standards within their validity time, with similarity above 0.999; in addition for the expired 9 batches of RI, their physical fingerprint spectra did not meet the standards. The results showed that physical fingerprint spectrum can be used for the quality control of RI, with a certain exemplary role in the quality evaluation of traditional Chinese medicine injection.

ABSTRACT

Our previous results have demonstrated that insulin reduces myocardial ischemia/reperfusion (MI/R) injury and increases the postischemic myocardial functions via activating the cellular survival signaling, i.e., phosphatidylinositol 3-kinase (PI3-K)-Akt-endothelial nitric oxide synthase (eNOS)-nitric oxide (NO) cascade. However, it remains largely controversial whether c-Jun NH2-terminal kinase (JNK) is involved in the effects of insulin on MI/R injury. Therefore, the aims of the present study were to investigate the role of JNK, especially the cross-talk between JNK and previously expatiated Akt signaling, in the protective effect of insulin on I/R myocardium. Isolated hearts from adult Sprague-Dawley rats were subjected to 30 min of regional ischemia and followed by 2 or 4 h of reperfusion (n=6). The hearts were pretreated with PI3-K inhibitor LY294002, or phosphorylated-JNK inhibitor SP600125, respectively, then perfused retrogradely with insulin, and the mechanical functions of hearts, including the heart rate (HR), left ventricular developed pressure (LVDP) and instantaneous first derivation of left ventricular pressure (+/-LVdp/dt(max)) were measured. At the end of reperfusion, the infarct size (IS) and apoptotic index (AI) were examined. MI/R caused significant cardiac dysfunction and myocardial apoptosis (strong TUNEL-positive staining). Compared with the control group, insulin treatment in MI/R rats exerted protective effects as evidenced by reduced myocardial IS [(28.9 +/- 2.0)% vs (45.0 +/- 4.0) %, n=6, P<0.01], inhibited cardiomyocyte apoptosis [decreased AI: (16.0 +/- 0.7) % vs (27.6 +/- 1.3) %, n=6, P<0.01] and improved recovery of cardiac systolic/diastolic function (including LVDP and +/-LVdp/dt(max)) at the end of reperfusion. Moreover, insulin resulted in 1.7-fold and 1.5-fold increases in Akt and JNK phosphorylation in I/R myocardium, respectively (n=6, P<0.05). Inhibition of Akt activation with LY294002 abolished, and inhibition of JNK activation with SP600125 enhanced the cardioprotection by insulin, respectively. And the abolishment by LY294002 could be partly converted by SP600125 pretreatment. In addition, SP600125 also decreased the Akt phosphorylation (n=6, P<0.05). These results demonstrate that insulin simultaneously activates both Akt and JNK, and the latter further increases the phosphorylation of Akt which attenuates MI/R injury and improves heart function; this cross-talk between Akt and JNK in the insulin signaling is involved in insulin-induced cardioprotective effect.
